Work Ready Employment Case Manager

About the Role

This position is part of Signal Centers' innovative workforce development program, which provides a unique combination of job training, paid internships, and intensive case management. The Employment Case Manager will provide intensive case management support to TANF-eligible individuals to prepare them for long-term, stable employment with benefits.

Responsibilities

• Job Coaching: Assist participants in securing part-time or full-time employment while continuing education and training.
• Intensive Model: Follow an intensive case management model, maintaining weekly contact with participants, employers, and education/training entities.
• Barrier Removal: Provide intensive support to eliminate or overcome barriers to success and collaborate with educators, trainers, and employers to troubleshoot issues.
• Case Management: Carry and maintain a caseload of 20-25 participants, including case notes, assessments, and data entry to track program progress and outcomes.
• Training: Participate in DHS-provided training on program support, TANF, and community resources.
• Community Outreach: Participate in community, partner, and outreach events as assigned.
• Assessment & Planning: Conduct initial and ongoing assessments of clients’ skills, experience, and employment barriers to develop personalized career plans.
• Job Readiness Training: Provide hands-on assistance with resume building, interview preparation, and job search strategies.
• Resource Coordination: Connect clients with essential community support, including housing assistance, transportation, and childcare, to eliminate barriers to work.
• Monitoring & Support: Conduct regular check-ins to track client progress, provide ongoing mentoring, and ensure successful long-term job retention.
• Documentation: Maintain accurate, up-to-date client files, service logs, and outcome reports to ensure compliance with funding and agency requirements.
• Other duties as assigned by the Director.

Requirements

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
• Experience working with inner-city families in a case management model or in workforce development.
• Valid Driver's License and reliable transportation with valid insurance per TN state requirements.
• Organized and proficient with keeping case management notes and data entry requirements.
• Professional demeanor and ability to represent the agency well in the community.
• Ability to work cohesively in a team environment.
