Responsibilities
- Working collaboratively with the interprofessional team to ensure an evidence-based plan of stroke care
- Provides in-person support for the clinical care of patients
- Expert Stroke resource for nurses and providers
- Captures and coordinates patient care using defined protocols
- Provides emotional support to patients and families
- Provides patient and family education
- Completes NIHSS
- Intervenes in real-time quality measures
- Conducts chart reviews
- Documents within patients’ electronic health record
- Collects and manages data for regulatory compliance and accreditation
- Provides EMS feedback
- Provides patient discharge follow-up
- Collaborates with healthcare team to create discharge plan
- Provides staff education
- Participates in quality initiatives and research
- Committee membership
- BASIC KNOWLEDGE:
- Graduate of an accredited school of nursing, associate’s degree in nursing required, bachelor’s degree in nursing preferred
- Current license to practice as a Registered Nurse in the State of Rhode Island
- BLS certification through AHA
- ACLS within six months of hire
- NIH Stroke Scale (NIHSS) certification within six months of hire
- Stroke certification within 2 years of hire
- Understanding of the provisions of the Nurse Practice Act of the State of Rhode Island
- Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages, and cultural patterns in each step of the care process
- EXPERIENCE:
- Experienced nurse with 3-5 years of acute care experience
- Stroke patient care experience strongly preferred
- Strong interpersonal skills to effectively function as a member of the healthcare team to coordinate and discuss patient care activities
