RN Patient Care Navigator - Stroke Program

About the role

Provide professional nursing care to support neurologists and multidisciplinary teams in managing stroke patients, focusing on acute stroke alerts, interventions, and timely care.

Responsibilities

• Conduct neurological assessments and follow treatment protocols
• Assist with patient transport
• Serve as a resource for stroke protocols, quality initiatives, and compliance
• Educate patients, families, and staff on stroke care
• Collaborate with the stroke team to meet center standards and maintain stroke program data

Qualifications

• Bachelor's Degree in Nursing
• Minimum three years of RN acute care experience with stroke patients
• Current Illinois RN licensure
• CPR (BLS) certification; ACLS certification within 3 months of hire
• NIHSS certification

Preferred Skills

• 3-5 years critical care or inpatient stroke care experience
• Calm demeanor under stress
• Strong communication skills
