RN Patient Care Navigator - Stroke Program

About the Role

Provide professional nursing care and coordinate stroke patient care throughout the continuum, supporting acute interventions and serving as a clinical resource for stroke protocols, quality initiatives, and regulatory compliance.

Responsibilities

• Perform neurological assessments and follow treatment protocols
• Assist with emergency testing and transfers
• Collaborate with multidisciplinary teams
• Maintain stroke program data
• Educate patients, families, and staff

Requirements

• Bachelor's Degree in Nursing
• Minimum three years of RN acute care experience in stroke treatment
• Valid Illinois RN license
• Current BLS and NIHSS certifications
• ACLS certification (or within 3 months of hire)

Preferred Qualifications

• 3-5 years of critical care or inpatient stroke experience
• Ability to maintain professionalism under stress
• Strong communication skills
