RN Patient Care Navigator - Stroke Program

Role: RN Patient Care Navigator – Stroke Program Responsibilities Provide professional nursing care to support neurologists, physicians, and APPs in managing stroke patients across the care continuum.Focus on acute stroke alerts, emergency interventions, and collaboration with multidisciplinary teams to ensure timely, evidence-based care.Perform neurological assessments, manage emergency protocols, and assist with patient transport and transfers.Serve as a clinical resource for stroke protocols, quality improvement, and regulatory compliance.Educate patients, families, and healthcare staff about stroke management and prevention.Assist in maintaining stroke program data and standards to ensure comprehensive stroke center compliance. Qualifications & Skills Bachelor's Degree in Nursing.At least three years of RN acute care experience in stroke treatment.Current Illinois RN licensure, BLS, and NIHSS certification.ACLS certification (or obtain within 3 months).Strong critical thinking, communication, and ability to remain calm under stress. Preferred Experience 5+ years in critical care or inpatient stroke care.Experience with acute stroke patient management.
