Position: RN Patient Care Navigator – Stroke Program Responsibilities: Provide professional nursing care to support neurologists, physicians, and APPs in managing stroke patients across the continuum. Focus on acute stroke alerts, emergency interventions, and collaboration with multidisciplinary teams to ensure timely, evidence-based care. Conduct neurological assessments, follow treatment protocols, facilitate patient transport, and assist with transfers. Serve as a clinical resource for stroke protocols, quality improvement, and regulatory compliance. Educate patients, families, and healthcare staff about stroke management and care standards. Qualifications: Bachelor's Degree in Nursing, at least three years of RN acute care experience in stroke treatment, current Illinois RN licensure, CPR (BLS), and NIHSS certification. ACLS certification preferred or to be obtained within three months. Strong communication skills, ability to remain calm during emergencies, and critical thinking are essential. Preferred Skills: 3-5 years of critical care or inpatient stroke care experience, experience with acute stroke patients, and professionalism under stress.
