RN Patient Care Navigator - Stroke Program

About the Role

Provide specialized nursing care to support neurologists and physicians in the stroke continuum, focusing on acute stroke alerts, emergency interventions, and multidisciplinary collaboration. Serve as a clinical resource for stroke protocols, quality improvement, and regulatory compliance.

Responsibilities

• Ensure timely, evidence-based care for stroke patients
• Perform neurological assessments and manage emergencies
• Assist with patient transport
• Support stroke education for patients, families, and staff
• Coordinate care and collaborate with the neurosciences team to uphold stroke program quality standards
• Maintain compliance with stroke care standards and protocols

Requirements

• Bachelor's Degree in Nursing
• Minimum three years of RN acute care experience with stroke patients
• Current Illinois RN licensure
• Current BLS certification; ACLS certification within 3 months if not already held
• NIHSS certification
