RN Patient Care Navigator - Stroke Program

Position: RN Patient Care Navigator – Stroke Program Responsibilities: Provide professional nursing care to support neurologists and physicians in managing stroke patients, focusing on acute stroke alerts, emergency interventions, and multidisciplinary collaboration. Ensure timely, evidence-based care, perform neurological assessments, follow emergency protocols, assist with patient transport, and serve as a resource for stroke protocols, quality improvement, and regulatory compliance. Educate patients, families, and staff about stroke care. Qualifications: Bachelor's Degree in Nursing, at least 3 years of RN acute care experience in stroke treatment, Illinois RN licensure, BLS, NIHSS certification, and ACLS certification (or within 3 months). Preferred: 3–5 years of critical care or inpatient stroke care experience, ability to remain calm under stress, and strong communication skills.
