RN Patient Care Navigator - Stroke Program

Responsibilities

• Provide professional nursing care to support neurologists and multidisciplinary teams in managing stroke patients throughout the stroke continuum.
• Focus on acute stroke alerts, emergency interventions, and timely, evidence-based care delivery.
• Perform neurological assessments, adhere to emergency protocols, and assist with patient transport and transfers.
• Collaborate with stroke coordinators and neurosciences teams to maintain stroke program standards and data collection.
• Educate patients, families, and staff about stroke care and protocols; serve as a clinical resource for quality improvement and regulatory compliance.

Qualifications & Skills

• Bachelor's Degree in Nursing.
• At least three years of RN acute care experience in stroke treatment.
• Current Illinois RN licensure, BLS, and NIHSS certification; ACLS certification within three months.
• Strong critical thinking, communication skills, and ability to remain calm under stress.
