Role: RN Patient Care Navigator – Stroke Program Responsibilities: Provide professional nursing care to support neurologists and physicians in managing stroke patients, focusing on acute stroke alerts, emergency interventions, and multidisciplinary collaboration to ensure timely, evidence-based care. Conduct neurological assessments, follow treatment protocols, assist with patient transport, and serve as a resource for stroke protocols, quality improvement, and regulatory compliance. Educate patients, families, and staff on stroke care. Qualifications & Skills: Bachelor’s Degree in Nursing, minimum three years of RN acute care experience in stroke treatment, current Illinois RN licensure, BLS, ACLS (or obtain within 3 months), NIHSS certification, strong communication skills, and ability to perform effectively during emergencies. Preferred experience includes 3-5 years in critical care or inpatient stroke care, with a calm demeanor in stressful situations. Key Duties: Coordinate stroke patient care, support acute interventions, ensure compliance with standards, and contribute to program data and quality initiatives.
