Role: RN Patient Care Navigator - Stroke Program Responsibilities Provide professional nursing care to support neurologists, physicians, and APPs in managing stroke patients throughout the continuum of care.Focus on acute stroke alerts, emergency interventions, and collaboration with multidisciplinary teams to ensure timely, evidence-based treatment.Perform neurological assessments, follow treatment protocols, and assist with patient transport and transfers.Serve as a clinical resource for stroke protocols, quality improvement, and regulatory compliance.Educate patients, families, and staff about stroke care. Qualifications Bachelor's Degree in Nursing.Minimum three years of RN acute care experience with stroke patients.Current Illinois RN licensure, CPR, and NIHSS certification; ACLS certification within three months.Strong communication skills and ability to remain calm under pressure. Preferred Skills 5+ years of critical care or inpatient stroke care experience.Experience with acute stroke patient management.Ability to maintain professionalism during stressful situations.
