RN Patient Care Navigator - Stroke Program

Position: RN Patient Care Navigator – Stroke Program Role & Responsibilities: Provide professional nursing care to support neurologists, physicians, and APPs in managing stroke patients. Focus on acute stroke alerts, emergency interventions, and collaborating with multidisciplinary teams to ensure timely, evidence-based care. Perform neurological assessments, follow treatment protocols, assist with patient transport, and serve as a resource for stroke protocols, quality improvement, and regulatory compliance. Educate patients, families, and staff about stroke care. Maintain stroke program data and ensure care standards meet comprehensive stroke center requirements. Qualifications & Skills: Bachelor's Degree in Nursing, at least three years of RN acute care experience in stroke treatment, Illinois RN licensure, current BLS, ACLS (or obtain within 3 months), and NIHSS certification. Preferred experience includes 3-5 years in critical care or inpatient stroke care, with strong communication skills and ability to manage stressful situations effectively.
