Social Worker - On Call Day Shift

Role Overview This position provides comprehensive social work services, including assessment, care planning, case management, and coordination of care for patients and their families. It involves conducting psychosocial evaluations, developing treatment plans, and delivering interventions to address emotional, mental, and behavioral health issues. The role also entails serving as a liaison between the healthcare organization and community resources to enhance service integration and foster community relationships. Key Responsibilities Prepare, evaluate, and implement social work service plans, including psychosocial assessments and treatment of individuals, couples, families, and groups.Manage cases by coordinating services, providing resource referrals, advocacy, and ensuring appropriate patient placement.Contribute to program development, policy implementation, quality improvement, and departmental activities such as staff training and audits.Conduct research and document findings in compliance with regulations and departmental standards. Qualifications & Skills Master's degree in social work with licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Hawaii.Basic Life Support certification from the American Heart Association.Experience working with medical diagnoses and knowledge of community resources.Strong networking, communication, and organizational skills. Preferred Qualifications Minimum of one year of social work experience.
