Emergency Department Social Worker - On-Call Day Shift

Role Overview

Provides psychosocial assessments and age-appropriate social medicine services as a key member of the emergency department/CDU care team. Responsible for identifying patient needs, developing care plans, and coordinating support to address issues such as illness adjustment, social support, abuse, neglect, and legal decisions.

Responsibilities

• Responds to care team referrals and conducts psychosocial assessments of patients.
• Assists patients and families with coping strategies and connects them with community resources post‑discharge.
• Supports discharge planning, ensuring appropriate placements and resources are in place.
• Provides social medicine consultations on abuse, neglect, advanced care planning, and legal matters.
• Participates in quality improvement, departmental activities, and staff development.

Qualifications & Skills

• Minimum one year of healthcare experience.
• Master’s degree in Social Work and licensed as a Social Worker or Clinical Social Worker.
• Knowledge of medical terminology, psychosocial impacts of illness, and community resources.
• Strong interpersonal, communication, and customer service skills.
• Experience working with interdisciplinary teams and understanding care systems.
