Emergency Department Social Worker - On-Call Day Shift

Job Summary Seeking a social worker to join the emergency department and CDU care team. Responsible for conducting psychosocial assessments, developing and implementing age-appropriate social medicine plans, and coordinating patient care to address needs such as new diagnoses, terminal illness, social support, and complex finances. Essential Responsibilities Provide social medicine expertise, responding to care team referrals and identifying high-need populations.Complete psychosocial assessments, document findings, and develop care plans aligned with regulations.Assist patients and support systems with coping strategies, discharge planning, and resource referrals.Conduct consultations for cases involving abuse, neglect, advance care planning, and legal decisions.Coordinate discharge processes, ensuring appropriate community placement and post-discharge support.Offer information, advocacy, education, and mediation services to patients and families.Stay current with social work practices through ongoing development and participate in quality improvement initiatives.Engage in departmental functions, team meetings, and orientation activities. Qualifications Minimum one year of healthcare experience.Master’s degree in Social Work.Licensed or Clinical Licensed Social Worker in Hawaii.Basic Life Support certification within six months of hire. Additional Skills Experience with interdisciplinary teams and knowledge of medical terminology.Ability to collaborate with healthcare professionals and patients.Understanding of care processes, community resources, and family dynamics.Strong customer service skills and proficiency in documentation and typing.
