Role Overview
Key Responsibilities
- Responds to referrals and identifies high-need populations, conducting psychosocial assessments and developing care plans.
- Affords patients and support systems with coping strategies, discharge planning, and community resource referrals.
- Conducts social medicine consultations on issues like abuse, neglect, advance care planning, and legal decisions.
- Supports coordination of discharge and placement in community settings, ensuring appropriate care transitions.
Qualifications And Skills
- Master's degree in Social Work and Licensed or Licensed Clinical Social Worker license.
- Minimum one year healthcare experience, with knowledge of medical terminology, interdisciplinary teamwork, and psychosocial impacts of illnesses.
- Strong communication skills, customer service orientation, and familiarity with community resources and care systems.
- Basic Life Support certification required within 0.5 months of hire.
