About the Role
Provides comprehensive social work services including assessment, care planning, case management, and coordination for patients and their families. Acts as a liaison between healthcare providers and the community to integrate services, improve relationships, and promote patient well-being.
Responsibilities
- Conducts psychosocial assessments, develops and evaluates social work care plans, and provides individual, family, and group interventions to address emotional, mental, and behavioral health needs.
- Manages cases by coordinating resources, offering referrals, advocacy, and education, ensuring appropriate service placement.
- Participates in program development, policy implementation, performance improvement, and quality assurance activities.
- Performs research, documents findings per standards, and maintains accurate clinical records.
- Engages in departmental functions, staff training, and orientation of new personnel.
Requirements
- Master’s degree in social work and licensure as a Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.
- Basic Life Support certification from the American Heart Association.
- Experience working with medical diagnoses and networking with community agencies preferred.
