Emergency Department Social Worker - On-Call Day Shift

Role: Provides psychosocial assessments and social medicine services as a key member of the emergency department/CDU care team. Responsibilities Responds to referrals and identifies high-need populations such as those with new diagnoses, terminal illnesses, homelessness, or complex social issues.Conducts psychosocial assessments, develops and documents care plans, and ensures timely record-keeping compliant with regulations.Assists patients and families with coping strategies, social support, and post-discharge planning, including resource referrals and community connections.Provides social medicine consultations for cases involving abuse, neglect, advance care planning, and legal decisions.Contributes to discharge planning, ensuring appropriate community placement and resource utilization.Engages in performance improvement, departmental activities, staff education, and quality assurance efforts. Qualifications Minimum one year of healthcare experience.Master’s degree in Social Work and valid licensure as a Licensed or Clinical Social Worker.Basic Life Support certification within six months of hire. Skills & Knowledge Experience working with interdisciplinary teams and understanding medical terminology.Ability to collaborate with healthcare providers, patients, and support systems effectively.Knowledge of care processes, community resources, family dynamics, and end-of-life issues.Strong customer service, organizational, and documentation skills.
