Emergency Department Social Worker - On-Call Day Shift

About the role

Provides psychosocial assessments and social medicine services as a key member of the emergency department/CDU care team.

Responsibilities

• Responds to referrals and identifies high-need populations such as those with new diagnoses, terminal illnesses, homelessness, or complex social issues.
• Conducts psychosocial assessments, develops and documents care plans, and ensures timely record-keeping compliant with regulations.
• Assists patients and families with coping strategies, social support, and post-discharge planning, including resource referrals and community connections.
• Provides social medicine consultations for cases involving abuse, neglect, advance care planning, and legal decisions.
• Contributes to discharge planning, ensuring appropriate community placement and resource utilization.
• Engages in performance improvement, departmental activities, staff education, and quality assurance efforts.

Qualifications

• Minimum one year of healthcare experience.
• Master’s degree in Social Work and valid licensure as a Licensed or Clinical Social Worker.
• Basic Life Support certification within six months of hire.

Skills

• Experience working with interdisciplinary teams and understanding medical terminology.
• Ability to collaborate with healthcare providers, patients, and support systems effectively.
• Knowledge of care processes, community resources, family dynamics, and end-of-life issues.
• Strong customer service, organizational, and documentation skills.
