About the Role
Provides psychosocial assessments and social medicine services as part of the emergency department/CDU care team, addressing the needs of patients and families in acute care settings.
Responsibilities
- Conducts psychosocial assessments, identifies needs, and develops care plans in response to referrals and case findings.
- Assists patients and support systems with coping strategies related to illness, including cases involving abuse, neglect, or legal decisions.
- Provides community resource referrals, supports discharge planning, and ensures appropriate patient placement post-discharge.
- Participates in social work consultations, performance improvement, and departmental activities, including staff education and audits.
Qualifications
- Master’s degree in Social Work and a valid license (LCSW or equivalent).
- Minimum one year of healthcare experience and knowledge of medical terminology and psychosocial impacts of illnesses.
- Strong interpersonal skills, experience with interdisciplinary teams, and familiarity with community resources and care systems.
Skills & Competencies
- Excellent communication, client advocacy, and resource referral skills.
- Ability to document interactions accurately and maintain compliance with regulations.
- Customer-focused service approach and knowledge of family dynamics and end-of-life issues.
