About the role

The Social Worker (BSW) at St. Mary's Medical Center will provide case management services to a selected group of patients. This role involves exercising independent judgment and professional discretion in collaboration with attending physicians, residents, and advocates for patients and families.

Responsibilities

• Collaborate with attending physicians and residents to support patients and families during the acute episode of care.
• Serve as an advocate to help patients and families navigate their healthcare journey.
• Maintain oversight of patients' continuing care to ensure appropriate resource utilization and facilitate safe and timely discharges.
• Utilize collaborative problem-solving skills to address the needs of patients and physicians.
• Provide comprehensive medical social work services as required, utilizing resources such as family, community, state, and federal agencies, as well as the patient's own resources, to aid in the patient's transition back into productive life.

Requirements

• Bachelor of Social Work (BSW) degree
• Current license to practice social work in the state of [State]
• At least 1 year of relevant clinical social work experience
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
