About the role
The Social Worker (MSW) at St. Mary's Medical Center is responsible for providing case management services to a selected group of patients. This role involves exercising independent judgment and professional discretion in collaboration with attending physicians, residents, and advocating for patients and their families during the acute episode of care.
Responsibilities
- Collaborate with attending physicians and residents to support patients and families through the acute episode of care.
- Monitor patients' continuing stay to ensure appropriate resource utilization and facilitate safe and timely discharge planning.
- Proactively use collaborative problem-solving skills to address the needs of patients and physicians.
- Provide a full range of medical social work services as required, utilizing resources such as family, community, state, and federal agencies, as well as the patient's own resources, to assist in the patient's return to a productive life.
Requirements
- Masters of Social Work (MSW) degree from an accredited program.
- Current New York State license as a Social Worker (LCSW).
- At least 2 years of recent relevant experience in a hospital setting.
- Experience working with medically complex patients and their families.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Qualifications
- Bachelor's degree in Social Work or related field.
- Current New York State license as a Social Worker (LCSW).
- At least 1 year of recent relevant experience in a hospital setting.
- Experience working with medically complex patients and their families.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
