About the role

The Social Worker (MSW) at St. Mary's Medical Center will exercise independent judgement and professional discretion in the practice of case management to a selected group of patients. They will collaborate with attending physicians and residents and serve as an advocate to help patients and families navigate the acute episode of care. The Social Worker will monitor patients' continuing stay for resource utilization and help prepare them for a safe and timely discharge. They will proactively use collaborative problem-solving skills to meet the needs of the patients and physicians. The Social Worker will provide a full range of medical social work services as needed and rely heavily on such resources as family, community, state and federal agencies as well as the patient’s resources to assist the patient’s return to a productive life.

Responsibilities

  • Collaborate with attending physicians and residents to support patients and families during the acute episode of care.
  • Serve as an advocate to help patients and families navigate the healthcare system.
  • Maintain ongoing monitoring of patients’ resource utilization and facilitate safe and timely discharges.
  • Utilize collaborative problem-solving skills to address the needs of patients and physicians.
  • Provide a full range of medical social work services as needed.
  • Leverage family, community, state and federal agency resources, as well as patient resources, to support the patient’s transition back to a productive life.

Requirements

  • Master of Social Work (MSW) degree from an accredited program.
  • Current New York State license as a Social Worker (LCSW).
  • At least 2 years of relevant clinical experience in a hospital setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in Microsoft Office Suite.
