About the role
The Social Worker (BSW) at St. Mary's Medical Center will exercise independent judgement and professional discretion in the practice of case management to a selected group of patients. They will collaborate with attending physicians and residents and serve as an advocate to help patients and families navigate the acute episode of care. The BSW will monitor patient’s continuing stay for resource utilization and help prepare them for a safe and timely discharge. They will proactively use collaborative problem solving skills to meet the needs of the patients and physicians. The BSW will provide a full range of medical social work services as needed and rely heavily on such resources as family, community, state and federal agencies as well as the patient’s resources to assist the patient’s return in a productive life.
Responsibilities
- Collaborate with attending physicians and residents to support patients and families during the acute episode of care.
- Advocate for patients and families to ensure they have access to necessary resources and services.
- Maintain ongoing monitoring of patients’ resource utilization and facilitate timely discharges.
- Use collaborative problem-solving skills to address the needs of patients and physicians.
- Provide a full range of medical social work services as needed.
- Leverage family, community, state and federal agency resources, as well as patient resources, to support the patient’s transition back into a productive life.
Requirements
- Bachelor of Social Work (BSW) degree
- Current license to practice social work in the state of [State]
- At least 2 years of relevant experience in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
