About the Role
Supports the Social Services/Admissions Departments in the pre-admission, admission, and discharge process. Serves as the backup to the Director of Admissions and works with social services to ensure the psychosocial needs of residents and patients are met.
Responsibilities
- Tracks pre-admission referral activity daily/weekly.
- Coordinates and reviews resident/patient electronic referrals and communicates with the Director of Admissions to complete assessments.
- Manages the electronic referral bed-offer process and obtains pre-authorizations from HMOs.
- Validates Midnight Census with BOM.
- Conducts tours of the facility with potential residents/patients and their family members.
- Assists the Director of Admissions with arrangements and confirmation of all admissions.
- Completes room assignment, roommate notification, and notifies departments of anticipated admission.
- Welcomes new residents and ensures contracts are completed on a timely basis.
- Completes Admission Agreement with resident/patient or responsible party in a timely manner.
- Ensures specialized equipment is ordered as indicated (reviews contract content for need to obtain prior approval from authorized payer).
- Ensures level of care approval is obtained as indicated and/or OBRA screen (PASSAR, MIMR) is completed.
- Completes records and documentation in accordance with company policy and state and federal guidelines.
- Helps resolve concerns of residents and/or family and participates in the Guardian Angel Program.
- Provides patient comfort by utilizing resources and materials; transporting patients; answering patients' call lights and requests; reporting observations of the patient to nursing supervisor.
- Primary backup for the Director of Admissions when out on PTO, FMLA, or the Director of Admissions position is open.
- Trains Social Services Coordinator on backup responsibilities and coordinates monthly "Job Shadowing" with the designated Social Services Coordinator backup.
- Receives Director of Admissions Responsibility Training and coordinates monthly "Job Shadowing" with the Director of Admissions.
- Protects organization's value by keeping patient information confidential.
- Services and protects the facility by adhering to professional standards, company policies and procedures, federal, state, and local requirements, and standards.
- Assists in the completion of care plans and psychosocial assessments.
- Coordinates care plan meetings with families.
- Coordinates, notifies, and documents room changes and roommate notifications when necessary.
Requirements
- Minimum high school diploma or equivalent required.
- One or more years of experience in healthcare education, marketing, or insurance, or commensurate education preferred.
- Must be familiar with medical terminology.
- Proficient in the use of personal computers.
