About the role
The IRIS Consultant supports adults, older adults, and individuals with disabilities in Wisconsin's IRIS program by providing case management, care coordination, and social work services to promote independence, self-determination, and community inclusion.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct regular contacts and visits
- Monitor health and safety
- Educate about IRIS resources
- Collaborate with community partners
- Maintain accurate documentation
- Assist with functional screens
Qualifications
- Bachelor's degree in related fields and one year of experience
- OR four or more years of direct service experience
- Current IRIS Consultant certification preferred

- Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued
