About the role

An IRIS Consultant supports adults, older adults, and individuals with disabilities in Wisconsin's IRIS program.

Responsibilities

  • Build trusted relationships with participants
  • Develop and update personalized IRIS Support Plans
  • Conduct monthly contacts and quarterly in-person visits
  • Promote self-determination and community inclusion
  • Monitor health and safety
  • Educate on program eligibility
  • Collaborate with community resources
  • Maintain accurate documentation
  • Assist with assessments

Qualifications

  • Bachelor’s degree in Social Work or related field with at least one year of experience, or
  • Four or more years of relevant direct service experience, or
  • Current IRIS Consultant status
