About the role
An IRIS Consultant supports adults, older adults, and individuals with disabilities in Wisconsin's IRIS program.
Responsibilities
- Build trusted relationships with participants
- Develop and update personalized IRIS Support Plans
- Conduct monthly contacts and quarterly in-person visits
- Promote self-determination and community inclusion
- Monitor health and safety
- Educate on program eligibility
- Collaborate with community resources
- Maintain accurate documentation
- Assist with assessments
Qualifications
- Bachelor’s degree in Social Work or related field with at least one year of experience, or
- Four or more years of relevant direct service experience, or
- Current IRIS Consultant status
