About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program. This role involves case management, care coordination, social work, and community-based support to promote independence and self-determination for participants.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct regular contact and in-person visits
- Monitor health, safety, and program outcomes
- Educate on IRIS eligibility and resources
- Collaborate with healthcare providers, community organizations, and resource centers
- Maintain accurate documentation and compliance
- Assist with functional screens
- Work independently within a support team
Qualifications
- Bachelor's degree in related fields
- Experience or equivalent experience working with individuals with disabilities or older adults
- Experience in case management, care coordination, or community services is preferred
