About the role
The IRIS Consultant supports adults, seniors, and individuals with disabilities through Wisconsin's IRIS Program, focusing on case management, care coordination, social work, and community-based support.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct regular contacts and visits
- Promote participant independence and self-determination
- Monitor health and safety
- Educate on program resources
- Collaborate with community partners
- Maintain accurate documentation
- Assist with assessments
Qualifications
- Bachelor's degree in relevant fields with at least one year of experience working with disabilities or older adults
- Four or more years of direct experience
- Current IRIS Consultant status in good standing
Preferred skills include case management, care coordination, disability services, and behavioral health expertise.
