About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS program.
Responsibilities
- Build trusting relationships with participants and develop individualized support plans
- Assist with service plan updates and conduct regular contact and in-person visits
- Promote participant choice, independence, and community involvement
- Monitor health, safety, and program outcomes
- Educate participants about eligibility and available resources
- Collaborate with healthcare providers, community organizations, and resource centers
- Maintain accurate documentation and ensure compliance standards
- Support annual functional assessments
- Work independently within a team
Qualifications
- Bachelor’s degree in relevant fields with at least one year of experience working with individuals with disabilities or older adults, or four+ years of direct service experience
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with technology and a valid driver’s license with reliable transportation
- Fluency in additional languages (e.g., Spanish, Hmong, ASL) is highly valued

This role requires travel throughout Milwaukee and surrounding counties, a valid driver’s license, and meeting safety standards.
