About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities in Wisconsin's IRIS program.
Responsibilities
- Build trusted relationships with participants and assist in creating personalized IRIS Support and Services Plans
- Conduct monthly contacts and quarterly in-person visits to promote self-determination, independence, and community inclusion
- Monitor health, safety, and program outcomes, and educate participants about eligibility and available resources
- Collaborate with community organizations, healthcare providers, and resource centers
- Maintain accurate case documentation and assist with functional screens
- Work independently within an interdisciplinary team
Qualifications
- Bachelor's degree in Social Work, Human Services, or related fields with at least one year of experience working with older adults or individuals with disabilities
- OR four or more years of direct experience in similar services
- OR current good-standing IRIS Consultant in Wisconsin DHS-approved exemption
Preferred Skills
- Case management, care coordination, disability services, long-term care, social work, and healthcare navigation experience
- Strong communication, cultural awareness, and ability to work independently
- Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued
Additional Requirements
- Valid driver's license, reliable transportation, and willingness to travel within the region
- Proficiency with technology and commitment to participant rights and dignity
