About the role
Responsible for supporting adults, seniors, and individuals with disabilities within Wisconsin's IRIS program.
Responsibilities
- Build trusted relationships with participants and develop personalized IRIS Support and Services Plans.
- Conduct monthly contacts and quarterly in-person visits to monitor health, safety, and program outcomes.
- Promote participant choice, independence, self-direction, and community involvement.
- Educate clients about IRIS eligibility, resources, and enrollment requirements.
- Collaborate with community organizations, healthcare providers, and resource centers.
- Maintain accurate documentation, complete compliance paperwork, and assist with functional screens.
- Work independently while coordinating with an interdisciplinary team.
Qualifications
- Bachelor's degree in Social Work, Human Services, or related fields with at least one year of supervised experience working with individuals with disabilities or older adults, or
- Four or more years of direct experience in similar services, or
- Current IRIS Consultant in good standing with Wisconsin DHS exemption.
