About the role
Supporting adults, older adults, and individuals with disabilities through case management, care coordination, social work, and community-based services.
Responsibilities
- Build trusting relationships with participants and develop individualized support plans.
- Assist with enrollment, eligibility, and resource education.
- Conduct monthly contacts and quarterly in-person visits to monitor health, safety, and outcomes.
- Promote participant choice, independence, and community inclusion.
- Collaborate with healthcare providers, community organizations, and ADRCs.
- Maintain accurate documentation and complete compliance requirements.
- Support annual functional screens and work independently within an interdisciplinary team.
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of relevant experience, or four or more years of direct service experience, or current IRIS Consultant in good standing.
- Experience in case management, disability services, aging, or healthcare preferred.
- Strong communication, relationship-building, and cultural competency skills.
- Ability to work independently, manage a flexible schedule, and use technology effectively.
- Bilingual abilities in Spanish, Hmong, Arabic, Russian, ASL, or other languages valued.
- Valid driver’s license, reliable transportation, and willingness to travel within the community.
