Social Services Consultant

About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.

Responsibilities

• Build trusted relationships with participants and develop individualized support plans
• Assist with service plan updates and conduct regular contacts and in-person visits
• Promote self-determination, independence, and community inclusion
• Monitor health, safety, and program outcomes
• Educate participants on eligibility and available resources
• Collaborate with community organizations, healthcare providers, and resource centers
• Maintain accurate documentation and complete compliance reports
• Support annual functional screens
• Work independently within an interdisciplinary team

Qualifications

• Bachelor’s degree in social sciences or related fields with at least one year of experience working with individuals with disabilities or older adults, or
• Four or more years of direct experience, or
• Current IRIS Consultant standing

Preferred Skills & Experience

• Case management, care coordination, disability services, long-term care, and social work experience
• Strong communication, relationship-building, and cultural awareness skills
• Proficiency with technology and ability to work independently
• Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued
