About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.
Responsibilities
- Build trusted relationships with participants and develop individualized support plans
- Assist with service plan updates and conduct regular contacts and in-person visits
- Promote self-determination, independence, and community inclusion
- Monitor health, safety, and program outcomes
- Educate participants on eligibility and available resources
- Collaborate with community organizations, healthcare providers, and resource centers
- Maintain accurate documentation and complete compliance reports
- Support annual functional screens
- Work independently within an interdisciplinary team
Qualifications
- Bachelor’s degree in social sciences or related fields with at least one year of experience working with individuals with disabilities or older adults, or
- Four or more years of direct experience, or
- Current IRIS Consultant standing
Preferred Skills & Experience
- Case management, care coordination, disability services, long-term care, and social work experience
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with technology and ability to work independently
- Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued

- Valid driver’s license and reliable transportation
