About the role
We are seeking a compassionate IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program. This role involves building relationships with participants, developing and updating individualized plans, conducting regular contacts and in-person visits, promoting self-determination, independence, and community involvement, monitoring health and safety, educating about program eligibility and resources, collaborating with community partners, maintaining accurate documentation, and supporting interdisciplinary teams.
Qualifications
- Bachelor’s degree in a related field with experience working with individuals with disabilities or older adults, or extensive direct experience in this area
