About the role

Esperanza Health Center is a multicultural Christian ministry providing holistic, affordable, bilingual healthcare to the Latino and underserved communities of Philadelphia. Our mission is to deliver high-quality, compassionate care in Jesus' name, regardless of ability to pay, addressing the physical, emotional, social, and spiritual needs of those we serve.

Key Responsibilities

• Meet with patients in person or by phone to assess social determinants of health and provide culturally appropriate resources
• Maintain accurate documentation in the Electronic Medical Record (EMR)
• Provide limited case management services for an assigned panel of patients living with HIV
• Conduct crisis intervention and referrals for patients experiencing domestic violence or child abuse
• Assist uninsured patients with enrollment in health insurance programs and follow up to address barriers
• Facilitate referrals to Early Intervention and Autism Evaluation services
• Work with children and their families to support linkage to needed resources and assist with navigating the educational system, welfare system, and other external agencies impacting health and well-being

Qualifications & Skills

• Bachelor’s degree preferred
• Bilingual in English and Spanish required
• Ability to work effectively in a fast-paced, team-based environment
• Strong communication, organization, and interpersonal skills
• Passion for innovative approaches to delivering high-quality social services
• Commitment to working in a Christian healthcare ministry grounded in compassion, dignity, and service
