About the Role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This role involves case management, care coordination, social work, and disability services to help participants maintain independence, achieve personal goals, and exercise self-direction.
Responsibilities
- Developing and updating individualized support plans
- Conducting regular participant contacts and visits
- Promoting community inclusion
- Monitoring health and safety
- Educating participants on program requirements and resources
- Collaborating with community partners
- Maintaining accurate documentation
Qualifications
- Bachelor's degree in social work, psychology, counseling, or related field
- Experience working with individuals with disabilities or older adults, or equivalent experience
Preferred Skills
- Case management and care coordination experience
- Healthcare navigation knowledge
- Cultural competence
- Bilingual candidates are encouraged to apply
