About the Role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This role involves case management, care coordination, social work, and disability services to help participants maintain independence, achieve personal goals, and exercise self-direction.

Responsibilities

  • Developing and updating individualized support plans
  • Conducting regular participant contacts and visits
  • Promoting community inclusion
  • Monitoring health and safety
  • Educating participants on program requirements and resources
  • Collaborating with community partners
  • Maintaining accurate documentation

Qualifications

  • Bachelor's degree in social work, psychology, counseling, or related field
  • Experience working with individuals with disabilities or older adults, or equivalent experience

Preferred Skills

  • Case management and care coordination experience
  • Healthcare navigation knowledge
  • Cultural competence
  • Bilingual candidates are encouraged to apply
