About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.
Responsibilities
- Develop and maintain trusted relationships with participants
- Create and update individualized IRIS Support and Services Plans
- Conduct monthly contacts and quarterly in-person visits
- Promote participant choice, independence, and community inclusion
- Monitor health, safety, and program outcomes
- Educate participants about IRIS eligibility and resources
- Collaborate with community organizations, healthcare providers, and support teams
- Maintain accurate case documentation and comply with standards
- Assist with long-term care functional screens when needed
Qualifications
- Bachelor’s degree in Social Work or related field with at least one year of experience working with individuals with disabilities or older adults, or four or more years of relevant experience, or current IRIS Consultant status
- Preferred experience in case management, care coordination, disability services, or long-term care
- Strong communication, relationship-building, and cultural competence skills
- Proficiency with digital tools and a commitment to participant rights
- Bilingual candidates in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged to apply
This role requires a valid driver’s license, reliable transportation, and the ability to travel within the service area.
