About the role

The IRIS Consultant supports adults, seniors, and individuals with disabilities through case management, care coordination, and community-based services within Wisconsin's IRIS Program.

Responsibilities

  • Develop and maintain trusting relationships with participants.
  • Create and update individualized IRIS Support and Services Plans.
  • Conduct monthly contacts and quarterly in-person visits.
  • Promote participant choice, independence, and community involvement.
  • Monitor health, safety, and program outcomes.
  • Educate participants on IRIS eligibility, resources, and enrollment.
  • Collaborate with ADRCs, healthcare providers, and community organizations.
  • Maintain accurate documentation, ensuring compliance and timely reporting.
  • Assist with annual functional screens when needed.
  • Work independently while collaborating with an interdisciplinary team.

Qualifications

  • Bachelor's degree in related fields with at least one year of experience, or four or more years of direct service experience.
  • Experience in case management, care coordination, disability, or long-term care is preferred.
  • Strong communication, relationship-building, and cultural competency skills.
  • Proficiency with digital devices and systems.
  • Ability to work independently with flexibility and travel within the community.
  • Bilingual skills in languages such as Spanish, Hmong, or ASL are highly valued.

  • Valid driver's license, reliable transportation, and satisfactory driving record.
