About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.
Responsibilities
- Develop and maintain trusting relationships with participants
- Assist in creating and updating individualized IRIS Support and Services Plans
- Conduct monthly contacts and quarterly in-person visits
- Promote participant choice, self-direction, independence, and community integration
- Monitor health, safety, and program outcomes
- Educate participants about eligibility, resources, and enrollment
- Collaborate with community organizations, healthcare providers, and resource centers
- Maintain accurate case documentation and compliance standards
- Support annual functional screens
- Work independently within an interdisciplinary team
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related field with at least one year of supervised experience working with individuals with disabilities or older adults, or four or more years of relevant experience, or current IRIS Consultant status
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with technology and a commitment to participant rights and dignity
- Bilingual skills in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued

- Valid driver’s license and reliable transportation
- Ability to travel within Milwaukee and surrounding areas and meet insurance standards
