About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.
Responsibilities
- Build trusted relationships with participants
- Assist in creating and updating personalized IRIS Support and Services Plans
- Conduct regular participant contacts and in-person visits
- Promote participant choice, independence, and community involvement
- Monitor health, safety, and program outcomes
- Educate participants on IRIS eligibility and resources
- Collaborate with community organizations, healthcare providers, and ADRCs
- Maintain accurate documentation and compliance
- Support annual functional screens
- Work independently within a team
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related field with at least one year of experience, OR
- Four or more years of direct experience working with individuals with disabilities or older adults, OR
- Current IRIS Consultant in good standing with Wisconsin DHS exemption
