Social Services Consultant

About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.

Responsibilities

• Build trusted relationships with participants
• Assist in creating and updating personalized IRIS Support and Services Plans
• Conduct regular participant contacts and in-person visits
• Promote participant choice, independence, and community involvement
• Monitor health, safety, and program outcomes
• Educate participants on IRIS eligibility and resources
• Collaborate with community organizations, healthcare providers, and ADRCs
• Maintain accurate documentation and compliance
• Support annual functional screens
• Work independently within a team

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field with at least one year of experience, OR
• Four or more years of direct experience working with individuals with disabilities or older adults, OR
• Current IRIS Consultant in good standing with Wisconsin DHS exemption
