Social Services Consultant - Help People Live Independently

About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program. This role involves case management, care coordination, social work, and community support to help participants maintain independence, achieve personal goals, and self-direct services. You will build trusting relationships, develop individualized care plans, conduct regular contacts and visits, and promote participant choice and community inclusion.

Responsibilities

• Monitor health and safety of participants
• Educate participants about resources and eligibility
• Collaborate with healthcare providers and community organizations
• Maintain accurate documentation
• Work independently while collaborating with an interdisciplinary team

Qualifications

• Bachelor’s in Social Work or related field with at least one year of experience working with people with disabilities or older adults, or four or more years of relevant experience
• Current IRIS Consultant status or Wisconsin DHS exemption preferred
• Valid driver’s license, reliable transportation, and ability to travel within the community
