About the role

We are seeking a Full-time Case Manager to support the Emergency Department, focusing on discharge planning, connecting patients with community resources, and ensuring positive outcomes.

Responsibilities

• Coordinating care from pre-admission to post-discharge
• Facilitating communication among healthcare teams and agencies
• Supporting patients and families through crisis intervention and education
• Developing individualized care plans and providing appropriate referrals
• Participating in care conferences and transitional rounding
• Advocating for patients and ensuring safe, effective care transitions

Qualifications

• Bachelor’s Degree in Nursing or Social Work from an accredited program
• Unencumbered RN license in Minnesota or social work credential (LSW or equivalent)
• Basic Life Support (BLS) certification within six weeks if applicable
