About the role

We are seeking a full-time Case Manager to support the Emergency Department team, focusing on discharge planning, connecting patients with community resources, and promoting positive patient outcomes. The role involves coordinating care from pre-admission to post-discharge, enhancing patient flow, and facilitating interdisciplinary communication. The candidate should be comfortable working in diverse healthcare settings and advocating for clients to resolve systemic issues.

Responsibilities

• Provide crisis intervention and coordinate healthcare programs
• Respond to cases of abuse or neglect
• Develop care plans by utilizing community resources
• Coordinate discharge planning and connect patients with community resources
• Enhance patient flow and facilitate interdisciplinary communication
• Advocate for clients to resolve systemic issues

Requirements

• Bachelor’s Degree in Nursing or Social Work
• Unencumbered RN license or LSW credential
• Relevant certifications such as BLS
• Demonstrated commitment to patient advocacy, teamwork, and professional growth
