Care Manager or Social Worker

About the role

We are seeking a full-time Case Manager to support the Emergency Department. This role is responsible for coordinating discharge planning and connecting patients with community resources to ensure safe, clear next steps for patients. The Case Manager facilitates communication among healthcare teams and stakeholders and evaluates outcomes through integrated clinical, financial, and utilization data.

Responsibilities

• Provide crisis intervention and psychosocial support.
• Assist families with understanding medical situations and accessing services.
• Respond to abuse, neglect, or violence according to policies and laws.
• Develop and implement care plans and make appropriate referrals.
• Participate in care conferences and transitional rounding.

Qualifications

• Bachelor’s Degree in Nursing or Social Work.
• Unencumbered RN license in Minnesota or Social Worker credential.
• Basic Life Support (BLS) certification within six weeks (for social workers).
