Care Manager or Social Worker

About the Role

We are seeking a full-time Case Manager to support an Emergency Department team, focusing on discharge planning, connecting patients with community resources, and ensuring positive patient outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge.
• Facilitate communication among healthcare disciplines and community agencies.
• Evaluate outcomes based on clinical, financial, and utilization data.
• Advocate for patients and resolve issues within the healthcare system.
• Provide crisis intervention and assist families in understanding medical situations.
• Develop care plans and provide referrals to social and healthcare services.
• Participate in care conferences and transitional rounding.
• Demonstrate professionalism, critical thinking, and effective communication.

Qualifications

• Bachelor’s Degree in Nursing or Social Work.
• Unencumbered RN license or Social Work credential as per state requirements.
• Basic Life Support (BLS) certification within six weeks if applicable.
