Care Manager or Social Worker

About the role

We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, connecting patients with community resources, and promoting positive patient outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge
• Facilitate communication among healthcare providers and agencies
• Advocate for patient needs
• Provide crisis intervention
• Develop care plans
• Provide referrals and education to patients and families
• Engage in collaborative problem-solving and critical thinking

Requirements

• Bachelor’s Degree in Nursing or Social Work
• RN license or Social Worker credential valid in Minnesota
• Basic Life Support (BLS) certification required within six weeks
• Effective verbal and written communication skills
• Professionalism, independence, and ability to work as part of a team
