About the Role

We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, connecting patients with community resources, and ensuring positive outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge.
• Facilitate interdisciplinary communication.
• Evaluate outcomes through clinical and utilization data.
• Promote early intervention.
• Provide crisis intervention.
• Assist families in understanding medical situations.
• Coordinate healthcare programs.
• Respond to abuse or neglect.
• Develop care plans.
• Connect patients with appropriate services.
• Advocate for patients.
• Participate in care conferences and transitional rounding.

Requirements

• Bachelor’s Degree in Nursing or Social Work.
• Unencumbered RN license in Minnesota (for Care Managers).
• Basic Life Support (BLS) certification (for Social Workers).
