Healthcare Care Manager or Social Worker

About the role

We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge.
• Facilitate communication among healthcare teams and community agencies.
• Advocate for patients.
• For Social Workers:
  • Provide crisis intervention.
  • Understand human behavior.
  • Respond to abuse or neglect.
  • Develop care plans with appropriate referrals.
• For Care Managers:
  • Integrate care across services.
  • Improve patient flow.
  • Evaluate outcomes based on clinical, financial, and utilization data.

Requirements

• Bachelor’s Degree in Nursing or Social Work.
• RN license in Minnesota.
• Basic Life Support certification (for social workers within six weeks of hire).
