We are seeking a Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge
• Facilitate interdisciplinary communication
• Evaluate outcomes using clinical and utilization data
• Advocate for patients within the healthcare system
• Provide crisis intervention
• Coordinate healthcare programs
• Respond to abuse or neglect
• Develop care plans
• Connect families with community resources
• Demonstrate professionalism, critical thinking, and effective communication

Qualifications

• For Care Manager: Bachelor's degree in nursing, unencumbered RN license in Minnesota
• For Social Worker: Bachelor's in Social Work, Basic Life Support certification
