RN Care Coordinator (Part Time, Days)

About the Role

Inpatient Care Managers are Registered Nurses responsible for ensuring accurate patient admission levels for billing and reimbursement, providing cost-effective clinical coordination in acute and emergency settings, and managing complex transition planning. They independently assess patient needs, develop and implement care transition plans, and serve as expert consultants and educators for healthcare teams. The role involves proactive documentation, resource utilization, and system improvement to optimize patient care and resource use.

Responsibilities

• Coordinate post-discharge care for timely and effective patient transitions.
• Perform thorough, timely patient assessments reflecting psychosocial and care needs.
• Develop collaborative care plans with patients, families, and healthcare teams.
• Resolve barriers to safe transfers through team conferences, ensuring quality and resource efficiency.
• Maintain ongoing contact with interdisciplinary staff and support services for safe transitions.
• Ensure regulatory and compliance standards are met within care processes.
• Perform additional duties as needed.

Requirements

• Minimum two years RN experience in Med/Surg, ICU, Emergency, Home Health, Skilled Nursing, Hospice, LTAC, or Rehab.
• Successful completion of RN program; high school diploma or GED required.
• Active RN license, with additional state credentials as required; BLS certification within 3 months.

Skills

• Strong communication, teamwork, assessment, documentation, and resource utilization skills.
• Ability to provide culturally competent care, navigate conflicts, and use technological devices effectively.

Preferred Qualifications

• Knowledge of hospital and internal resource utilization, Medicare/Medicaid regulations, and utilization management.
• Certified in Case Management, BSN or higher degree preferred.
